Include only the operational facts needed for review: date or times, general property type, conditions found, actions taken, unit labels or counts, and outcomes.

Sensitive-data boundary — do not enter: rosters, CAPIDs, exact addresses, names, phone numbers, email addresses, incident or case identifiers, PHI or patient information, medical details, or operational secrets. Add required protected details only inside your authorized NERIS or RMS system.
